Notice

1. Complete all of the forms before the appointments (new clients only)
2. Masks are required
3. Upon arrival, remove your shoes and sanitize your hands
4. Late Arrival for Appointment
   For any late arrival of the appointment will continue the treatment with the remaining of your appointment time and a FULL Amount of your original booking will be applied.
   More than 15 minutes to the appointment, there is a possibility your time slot will be forfeited. We may not be able to accommodate you. In this case, the same cancellation fee of NO SHOW will be applied
5. Late Cancellations
   Rescheduling/Cancel 48 hours prior – penalty charges of 75% treatment fee
   Rescheduling/Cancel within 24 hours – penalty charges of 100% treatment fee (FULL AMOUNT)
6. No Show
   Client will be charged 100% (FULL AMOUNT) for a NO SHOW (missed) appointment. A no-show is when a patient misses an appointment without cancelling.We appreciate your understanding.
